Confirmation Guidelines

IMPORTANT: ASMPH has a limited number of YL-5 positions for SY 2025-2026.  These will be filled in the order of confirmations that are received. The Confirmation Link will be closed when the positions have been filled.

STEP 1: ACCEPTANCE OF OFFER 
(6 to 9 May 2025 only for Accepted Applicants)

Accepted applicants who wish to confirm their slot should fill out the following Google Form:

Applicants who have completed the ACCEPT OFFER FORM must print and sign the following contracts and submit them on 12-13 May 2025.

STEP 2: ISSUANCE OF STUDENT ID NUMBER  

The ASMPH Admissions Office will be sending an acknowledgment email for those who have accepted the offer for a YL-5 position. The acknowledgment email will include the applicant’s assigned student number and instructions for the payment of fees. 

Issuance of student numbers is done at the end of each business day. As such, please expect a confirmation email the next business day from the time the Accept Offer form was completed. 

STEP 3: PAYMENT OF FEES

Applicants confirming their slots must pay the following:

  1. THIRTY-FIVE THOUSAND PESOS (PHP 35,000.00)  non-refundable Confirmation Fee. This will be deducted from your tuition fee due for the first semester SY 2025-2026.
  2. SEVENTEEN THOUSAND EIGHT HUNDRED SIXTY NINE PESOS AND NINETY FIVE CENTAVOS (PHP 17,869.95) as full payment for the Transition Module Fee.* ALL incoming Year Level 5 (First Year Medicine Proper) students are required to attend the entire Transition Program. 
  3. For Ateneo de Manila University College Students/Graduates: THREE HUNDRED PESOS (PHP 300.00) as payment for Ateneo de Manila University College Official Transcripts with documentary stamps. Please note that payment for this fee will require a separate payment transaction using a different reference number. 

Applicants who have paid the confirmation and other fees must upload the proof of payment in the following Google Form: CONFIRMATION RECEIPT

STEP 4: PROCEED TO ASMPH ADMISSIONS OFFICE 
(on 12-13 May only, 9:00 am to 3:00 pm) 

Applicants must go to the ASMPH Admissions Office for the following: 

  1. Submission of lacking application requirements, if any 
  2. Submission of signed contracts listed in STEP 1 
  3. Picture-taking for the Office Transition Module and School IDs
  4. Measurement taking for the official white school uniform
    For your guidance, here are the estimated uniform rates:
    PhP 2,200 per set 
    Details of the Scrub Suit and Nameplate are to be announced accordingly. 

CONFIRMATION POLICIES AND OTHER INFORMATION

  1. CONFIRMATION REQUIREMENTS 
    Please prepare the following confirmation requirements to be submitted to the ASMPH Admissions Office on May 12-13, 2025:
    1. Lacking documents required for application, if any (e.g ORIGINAL Transcript of Records or ORIGINAL Grade Reports reflecting Final Grades of the required courses completed, etc.) 
    2. Certificate of Good Moral Character (GMC), must be dated and issued after the applicant’s date of graduation. Applicants graduating 2nd Semester may defer the submission of the GMC after graduation. 
    3. Birth Certificate and Citizenship Documents, if not yet submitted 
      • FILIPINOS BORN IN THE PHILIPPINES:
        ORIGINAL Birth Certificate on Security Paper issued by the National Statistics Office/Philippine Statistics Authority
      • IF BORN IN THE PHILIPPINES BUT 1 PARENT IS FILIPINO AND 1 PARENT IS FOREIGN (according to the Birth Certificate of the Student):
        A photocopy will be accepted provided that the original document will be presented for verification.
        • ORIGINAL Birth Certificate on Security Paper
        • Proof of Filipino citizenship:
          1. Philippine passport,
          2. Identification Certificate of Recognition (ICR),
          3. Bureau of Immigration Order
        • Other valid passport/s
      • FILIPINOS BORN ABROAD:
        Photocopy will be accepted provided that the original document will be presented for verification.
        • Birth Certificate issued by the country you were born in
        • Report of Birth of Child Born Abroad of Filipino Parent/s on Security Paper issued by the National Statistics Office
        • Proof of Filipino citizenship:
          1. Philippine passport,
          2. Identification Certificate of Recognition (ICR)
          3. Bureau of Immigration Order
      • FOREIGN STUDENTS:
        Photocopy will be accepted provided that the original document will be presented for verification.
        • Birth Certificate issued by the country you were born in
        • Foreign passport/s
        • Alien Certificate of Registration (ACR) or I-card (renewed yearly)
      • IF HOLDER OF DUAL CITIZENSHIP AND 1 OF THE CITIZENSHIPS IS FILIPINO:
        Photocopy will be accepted provided that the original document will be presented for verification.
        • Birth Certificate issued by the country you were born in
        • Passports or recognition papers of BOTH citizenships
      • IF HOLDER OF DUAL CITIZENSHIP AND BOTH CITIZENSHIPS ARE NON-FILIPINO:
        Photocopy will be accepted provided that the original document will be presented for verification.
        • Birth Certificate issued by the country you were born in
        • Foreign passport/s
        • Alien Certificate of Registration (ACR) or I-card (renewed yearly)
    4. IF MARRIED, Marriage Certificate on Security Paper issued by the National Statistics Office (Photocopy will be accepted provided that the original document will be presented for verification). 
       
  2. REFUND POLICY 

    NON-REFUNDABLE Fees:
    The following fee/s are non-refundable:

    • Confirmation Fee - In instances where incoming students shall no longer pursue their studies at ASMPH, the confirmation fees paid shall no longer be refundable.
       

    Refund Policy for Intersession Classes:1
    Applicable Intersession Classes of ASMPH are as follows:

    • Transition Module The following are the applicable refund schedules for students who have paid  in full for the Intersession classes:
If a student withdraws, in writing, at least one (1) month before the start of classes  100% refund of transition module fees paid
If a student withdraws, in writing, less than one (1) month before the start of classes 50% refund of transition module fees paid
If a student withdraws when the Transition Module has started No refund applicable

Students who have not paid the intersession fees in full are not entitled to a refund.
