One-time Health Declaration (OSS Memo)

In line with the Memo of the University President last 30 July 2022 about Updated Guidelines for Health Protocols & Campus Access (https://www.ateneo.edu/document/2022/07/30/update-no2-guidelines-health-protocols-campus-access-memo-u2223-013), which states:

“The Daily Health Declaration will no longer be required for campus access. In lieu of this form, all students and employees of the university are required to submit a one-time Declaration and Undertaking.”

All students of the Loyola Schools are advised to accomplish their one-time Health Declaration by filling up this form - https://go.ateneo.edu/health-undertaking-students-ls

Your one-time Health Declaration is required for continued campus access. Please note that if there are any changes in your health status, or you feel unwell for whatever condition, you must not proceed to school. If tested positive for CoVid-19 or any other serious disease, please report this to the Office of Health Services at healthservices@ateneo.edu. Deadline for the submission of the one-time Health Declaration form is on Saturday, August 13, 2022.
