FAQs about Submission of Chest X-ray and Immunization

1. What are the required immunization/vaccinations that you need to submit?
List of required Immunizations according to the DOH Expanded Program for Immunization:
- Hepatitis B (Documentation of at least three doses)
- Measles-Mumps-Rubella (MMR 2 doses)
- Chicken Pox (2 doses)
- Tetanus-diphtheria Booster(Last dose not more than 10 years ago)
- COVID-19 vaccination (optional: 1-2 doses, depending on the brand of vaccine, Booster if available)
2. What should I do if my immunizations are not complete?
If any of the required immunizations are not complete at the time of submission, please request a schedule for completion from the doctor and submit the certified schedule prior to the said deadlines. If these vaccines have been accomplished but certification is unavailable, you may submit results of serologic tests for the specific antibodies which may be done at your physician’s request.
3. What should I do if i cannot retrieve my immunization records?
If you don't have a copy of your childhood immunizations, try to ask your pediatrician. If your pediatrician is no longer reachable or has no record of your immunizations, please ask your current physician for a request for serologic tests to determine if prior vaccinations will yield adequate antibody levels. You may secure a physician's request and have these done at your preferred laboratory. You may upload these results to the BluPHR.
4. Is there a required date or validity of the Chest X-ray that you need to submit?
Chest X-ray must have been taken 6 months from the submission date. For any abnormal findings, clearance from your attending physician is required.
5. Where can I submit immunization and Chest X-ray requirements?
