Updating of Medical Records for First Semester SY 2023-2024 (OHS Memo)

Dear LS Students,
This is a friendly reminder to update your electronic medical records at the BluePHR system after the release of grades (July 28, 2023) along with the requirement for updating your AISIS records before the coming enrollment.
Updating your health record is required every semester prior to enrollment (see Health Requirements prior to enrollment).
Failure to update your health record will result in a hold on your AISIS account access that may interfere with enrollment procedures. Please enter any new health data if relevant eg. recent vaccinations, recent medical condition, immunization, or newly prescribed medications through the link: https://bluepass.ateneo.edu. Once you have updated your data, click "submit" at the end of the health record. Note that if you have no updates you will still need to click submit at the end of your record to signify that you have reviewed your data.
For students who have any current medical or psychological conditions, a recent certificate will be required for each semester. Note that failure to submit recent certification of a current condition will be taken as an indication that no assistance will be needed for the said condition for the semester.
