Subject: Medical record requirements
To: Undergraduate and Graduate
Dear Loyola Schools students,
We are glad to update you that the medical health record system, Blue PHR has been updated to serve you better. In particular, you can now access the Blue Pass portal which you will use to generate a QR code for campus entry for students who will be on campus for face-to-face classes.
As you settle into the new semester, please update your personal health record through the link: https://lsohs.bluphr.ph/ . Updating your personal medical records is required for all students in view of the CHED requirement for reporting vaccination status in schools. If you have a recent vaccination or booster shot, you will need to upload a copy of this. If you have no updates kindly indicate this by acknowledging that you verify your record to be updated at the end of the
form. Updating of medical records will be due every semester. Updating should be accomplished by March 14, 2022 to avoid a hold order on your AISIS account.
For those who have still not created a medical record, please do so at the link provided above before March 14, 2022. Please note that the Blue Pass is a requirement for all students. It is your link to the LS Office of Health Services online medical services.
